The Bone & Joint Center has partnered with Sharecare to fulfill requests for medical records.

How Do I Request My Medical Records?

If you are a patient or legal guardian with the appropriate legal documentation, please complete the online Patient Access Form to request a copy of your medical records. You will need a valid email address and government-issued ID.

How Do Third Parties Request Medical Records?

Healthcare providers, attorneys, insurance companies, and other third parties requesting records should use the Requester Submission Portal to submit their request and the patient’s authorization.

When Will My Online Medical Record Request Be Ready?

Your records will be ready in 3 – 5 business days from the receipt of your electronic request.

Paper Medical Records Requests

If you are unable to complete the online form, please download and complete the paper authorization form. The form must be legible and complete. Illegible or incomplete authorizations will be returned. Please note that records can typically be processed faster when requested online.

Records requested using the paper form are usually available within 10 days after the request is received. If you are picking up your records in person, please bring a government-issued ID. If someone else will pick up your records, please indicate this on the authorization form. That individual will also need to provide a government-issued ID.

Only the patient, parent/legal guardian, or the patient’s legal health care representative may sign the authorization form to release medical records. If you are requesting records on behalf of a patient or as the patient’s representative, please provide the appropriate legal documentation, such as an Advance Directive, Durable Power of Attorney for health care, or Conservatorship.
